1. Download the Workers’ Comp Claims Kit. Read and follow the instructions.
2. Per the Claims Kit instructions, call us to report the claim immediately at 808-527-7711.
3. Note: When you file your claim with us, we transmit your WC-1 to the DOL. Please do not mail your form directly.
